ADHD and ASD

Waiting times for ADHD and ASD services remain long, with waits for local providers generally exceeding 12 months and, in most cases, significantly more. This is an ongoing issue that is reflected nationally and reflects significant increases in demand over the past 3-4 years.
Lancashire and South Cumbria are seeking to address these challenges though development of an all-age neurodevelopmental pathway, which will be informed by broader work undertaken by NHSE on a national level.
This includes significant transformation for children and young people’s services, with a key focus on digital triage and a needs-based model. Adult services are also subject to wide-reaching transformation, with a full review of clinical pathways and implementation of new ways of working, including support through the voluntary sector and GP-led assessments.
We are also pursing an commissioning model whereby providers of services under right to choose can apply to be accredited and deliver services under an LSC contract. We expect this will enable improved waiting times, while ensuring services are being delivered against a locally-produced specification.
We would ask that you manage expectations of patients seeking referral for these conditions and choose alternative pathways wherever possible. This may include referral into core mental health services, including NHS Talking Therapies, but only where there is evidence of mental health concerns within the wider clinical presentation.
There remain ongoing national shortages in certain medications for ADHD, which is linked to the increases in demand for ADHD services and we expect to continue into 2025. Updates can be found at Prescribing available medicines to treat ADHD.
If you encounter this issue with one of your patients, in the first instance please consult the SPS site Prescribing and switching between modified-release methylphenidate.
In such instances that an equivalent medication is not available, please refer to the specialist provider, who would be expected to support transition to an available product. We would not expect primary care clinicians to oversee titration to a new product.
We are receiving numerous queries in relation to Patient Choice and wish to offer some guidance that we hope will support you in helping your patients.
Referrals under patient choice can be made by a GP, dentist, or optometrist and is based on the premise that a patient can choose their secondary care provider deemed clinically appropriate for the patient.
Both ADHD and ASD services are applicable to Patient Choice, and where referral to a non-commissioned provider is deemed clinically appropriate, and that patient is subsequently accepted into the service, Lancashire and South Cumbria ICB will fund care on an implied contractual basis.
We are aware of some patients believing that they have a legal right to be referred to a given service, but this is incorrect. The patient can exercise the right to choose an alternative provider to those commissioned locally, but any decisions relating to clinical appropriateness remain with the referrer, and patients should meet all necessary screening thresholds before being considered for referral.
For clarity, clinical judgement overrides patient choice is all circumstances. This has been confirmed in writing by NHSE, and if you are not completely assured that a provider can deliver the appropriate pathway for your patient, you are within your rights to decline to make that referral.
Examples of when this may apply include if a provider doesn’t offer ECGs, they don’t offer annual reviews, you believe an alternative clinical pathway would be more appropriate (e.g. NHS Talking Therapies), or you do not believe there is significant impairment associated with ADHD symptoms.
Lancashire and South Cumbria ICB do not maintain oversight of service delivery for providers commissioned by other ICBs, and issues relating to quality are generally under the responsibility of the host ICB.
The commissioner’s responsibility is limited to ensuring an NHS standard contract is in place for that service, and that the provider can deliver the service detailed within the contract. The level of due diligence is therefore less than would be undertaken in a normal procurement process.
We will endeavour to list all providers who have provided valid contracts for delivering care under Patient Choice, but this must not be seen as confirmation that such providers are clinically appropriate.
This judgement remains with the referring clinician, and if that clinician is not suitably assured of a particular provider, they are entitled not to make that referral.
The ICB maintain the right to decline payments if insufficient information is provided by a provider to demonstrate that they can deliver the given service(s), or if upon review, a contract is not considered acceptable under Patient Choice legislation.
We do however endeavour to work with providers to address any concerns raised through this process, and in most cases, are able to find a solution.

Only providers commissioned by LSC ICB are subject to use of the LSCMMG shared care agreement. While we would expect providers commissioned by other ICBs to deliver care within NHSE Shared Care guidelines, we are aware of some variations that differ from local arrangements.
If you experience any challenges with shared care relationships fore providers delivering care under Patient Choice, please escalate to LSC ICB at lscicb.mentalhealth@nhs.net for consideration.
Please note that Modality LLP are a locally commissioned provider and hence work to protocols defined within a LSC specification. This includes use of the locally developed shared-care agreement.
We ask that patients prescribed through the service are considered for shared care where clinically appropriate. Decisions not to accept shared care however will be respected as per Shared Care guidelines.
We have seen a considerable increase in patients self-funding assessment for ADHD, and in some cases prescribing, before requesting commissioned services to take over care and bypass agreed clinical pathways.
There are some instances where we have advised providers to accept existing diagnoses, notably when a patient moves into the local footprint and previously received NHS-funded care, but in most cases we have made the decision not to recommend accepting private assessments for care in a shorter-time frame than otherwise would have been the case had they accessed the local pathway.
We are of the view that such practice increases health inequalities for people who are unable to afford such measures, and we would encourage you to advise patients of this decision.
Shared care where care is privately funded is not recommended by the BMA (General practice responsibility in responding to private healthcare). Such practice could be particularly problematic if a patient or parent ceases funding ongoing care, leaving that patient without secondary care oversight, and clinical accountability entirely with the GP.
If you are unsure as to whether a request for shared care is for self-funded private care, or is being delivered under Patient Choice / right to choose (as some providers deliver both), please check whether a referral was made by your practice. If this is not the case, we would encourage you not to accept this arrangement, and to perform this check for all shared cared requests.
LSC ICB receive regular queries from primary care in attempting to understand the difference between Patient Choice providers and “private providers”.
There is often confusion that non-NHS providers are considered “private providers” and thus not applicable to Patient Choice. This matter can be particularly confusing when a provider delivers care both privately and for NHS patients.
Patient Choice covers all providers, whether NHS or non-NHS, who hold an NHS standard contract with an ICB in England. This differs from care that is funded by the patient directly, or some other non-NHS funding mechanism, which does not fall under the scope of Patient Choice legislation.
We are aware of requests for GPs to enter into shared care arrangements for privately funded care. We have received some reports from primary care colleagues of providers making requests for shared care on documentation that would suggest an NHS-funded arrangements (e.g. NHS letterhead), despite such requests relating to private arrangements.
We would ask you to be diligent with such requests and escalate any concerns to the ICB for consideration.
ADHD services are generally commissioned to deliver diagnostic assessment and any necessary treatment. Certain providers however are only commissioned to deliver assessments for ADHD, or in some cases, will only undertake treatment if the GP agrees to shared care at the point of referral.
We have received several queries in relation to this, with some instances of patients not being aware of the limitations of a referral until their care has commenced. This has led to those patients being unable to access necessary treatment following assessment.
NHSE have confirmed that assessment-only models are supported by Patient Choice. It should therefore be communicated to patients that in accepting an assessment-only service, they may not be able to access medication-based treatment without referral into another service, which would be subject to normal waiting times.
We do have have further ongoing issues with some providers not being set up to prescribe for patients outside of their locally commissioned footprint. We have worked extensively to resolve this issue, including engagement with NHSBSA, and following discussion have agreed to change the data they provide for assurance purposes.
Some providers remain unable to prescribe as we work through this issue, but we expect this to be resolved within the next few months. Details are noted in the provider section of this page.
If you are unsure whether a provider can deliver a full care pathway, please check with that provider before making a referral. We would recommend that unless the referring clinician is confident that an assessment-only model is clinically appropriate, we would not encourage referral.
Local NHS trusts commissioned to deliver ADHD services will generally provide ECGs where necessary.
Arrangements with Modality however, who are commissioned to deliver adult ADHD services across Lancashire and South Cumbria, are such that ECG requests will be signposted to the patient’s GP for completion.
Where this takes place, a £50 payment will be made to the practice upon completion. Forms for invoicing are included within the referral and fulfilled by Modality upon request.
The GP is entitled to decline this request, but in such instances, the patient would be required to travel St Helens to undertake the test, which is where the service is physically based.
Many providers of ADHD services under Patient Choice do not have ECG included within their contract. This should be considered when making a referral for any patient who may be deemed at risk of cardiovascular issues.
We will try to provide information on which providers have this function, but can only provide full assurances for locally-commissioned providers and the responsibility for this remains with the referring clinician.
Listed below are providers that NHS Lancashire and South Cumbria ICB has directly commissioned and hold a contract to deliver ADHD and/or ASD services to residents with a Lancashire and South Cumbria registered GP.
Due diligence has been undertaken against each of these providers and monitoring against quality standards is in place. Assurances can be provided that each provider algins to local agreements, including shared care guidance.
